세계보건기구(WHO)와 유니세프(UNICEF)가 최근 발표한 새로운 보고서에 따르면 2030년까지 세계 수십억 명이 안전하게 관리되는 가정용 식수, 위생시설, 위생 서비스에 접근할 수 없게 된다. 또 ‘코로나19’ 전염병 확산에도 전 세계 인구 10명 중 3명은 집에서 비누와 물로 손을 씻지 못한 것으로 나타났다.

WHO와 유니세프는 SDGs 6(여섯 번째 지속가능개발목표)인 ‘깨끗한 물과 위생’의 진척 상황을 파악하고 독려하기 위해 ‘2000〜2020년 가정용 식수, 위생시설, 위생 부문의 진척(Progress on household drinking water, sanitation and hygiene, 2000〜2020’이라는 보고서를 최근 발간했다. 그 내용을 소개한다.

SDGs(지속가능개발목표) 시행 5년
Five years into the SDGs

2030년 지속가능 개발을 위한 의제는 ‘세계를 지속가능하고 회복탄력적인 방향으로 전환’, ‘모든 사람의 인권 실현’, ‘모든 형태의 빈곤 종식’, ‘누구도 소외되지 않도록 보장’을 위해 유엔(UN) 회원국들이 과감하고 혁신적인 조치를 취할 것을 약속한 것이다. 2000년 들어 수십억 명의 사람들이 식수와 위생시설, 위생 서비스를 이용할 수 있게 됐지만, ‘모두를 위한’, ‘보편적’ 접근을 달성하고자 하는 지속가능개발목표(SDG)를 완전히 실현하려면 아직 갈 길이 멀다.

세계보건기구(WHO)와 유니세프(UNICEF)는 물, 위생시설 및 개인위생에 대한 공동 모니터링 프로그램(JMP) 보고서 ‘가정용 식수·위생시설·(개인)위생 부문의 진척(Progress on household drinking water, sanitation and hygiene)’에서 2030년까지 SDG 목표 달성을 위한 현재 진행률과 전망을 평가했다.

WHO와 유니세프에 따르면, SDGs를 시행한 지 5년이 지났지만, SDG 6.1(모두를 위한 안전하고 저렴한 식수에 대한 보편적이고 공평한 접근 보장) 및 6.2(여성과 소녀 및 취약계층의 요구에 특별한 주의를 기울이면서, 모두를 위한 적절하고 공평한 공중위생 및 개인위생에 대한 접근 달성 및 노상 배변 근절) 달성을 위한 노력은 제대로 진행되지 못하고 있다.

2030년까지 보편적인 적용범위를 달성하려면, 안전하게 관리되는 식수 서비스, 위생 서비스 및 기본 위생 분야에서 목표 달성을 위한 이행 속도를 4배 올려야 한다. 최빈개도국은 여전히 갈 길이 멀고 취약한 형편 속에서 진전을 가속화하기는 특히 어려울 것이다. 점점 더 많은 나라들이 농촌 지역을 비롯해 소외될 가능성이 큰 빈곤층 및 취약계층에 대한 서비스를 확장하는 데에 어려움을 느끼고 있다.

The 2030 Agenda for Sustainable Development commits UN member states to take bold and transformative steps to ‘shift the world onto a sustainable and resilient path’, ‘realize the human rights of all’, ‘end poverty in all its forms’, and ensure ‘no one will be left behind’. Since 2000, billions of people have gained access to drinking water, sanitation and hygiene services, but many countries still have a long way to go to fully realize the Sustainable Development Goal (SDG) ambition to achieve ‘universal’ access ‘for all’.

The WHO/UNICEF  Joint Monitoring Programme for Water Supply, Sanitation and Hygiene (JMP) report Progress on household drinking water, sanitation and hygiene assesses current rates of progress and the prospects for achieving SDG targets by 2030.

 Five years into the SDGs, the world is not on track to achieve SDG targets 6.1 and 6.2. Achieving universal coverage by 2030 will require a quadrupling of current rates of progress in safely managed drinking water services, safely managed sanitation services, and basic hygiene. Least Developed Countries have the furthest to go and it will be especially challenging to accelerate progress in fragile contexts. Many more countries are facing challenges in extending services to rural areas and to poor and vulnerable populations who are most at risk of being left behind.

2020년 기준 세계 인구의 74%(약 58억 명)가 안전하게 관리되는 식수 서비스를 이용했다. 이는 138개 국가 및 8개 SDG 지역 중 4개 지역에 대한 국가 추정치를 이용한 결과다. 이들 국가 및 지역에 거주하는 인구는 세계 인구의 45%를 차지한다. 안전하게 관리되는 식수 보급률은 도시지역(86%)보다 농촌지역(60%)에서 낮았고, 이 중 농촌에 거주하는 인구가 전체의 3분의 2에 달했다. 2020년까지 총 84개국이 최소한의 기본적인 식수 서비스에 대한 보편적 적용범위(99% 이상의 보급률)를 달성했다.

 
In 2020, 74 per cent of the global population used safely managed drinking water services. National estimates were available for 138 countries and four out of eight SDG regions, representing 45 per cent of the global population. Coverage was lower in rural areas (60 per cent) than in urban areas (86 per cent), which were home to two out of three of the 5.8 billion people using safely managed services. By 2020 a total of 84 countries had achieved universal (>99 per cent) coverage of at least basic drinking water services.

2020년 기준 세계 인구의 54%(42억 명)가 안전하게 관리되는 위생 서비스를 사용했다. 120개 국가와 8개 SDG 지역 중 7개 지역에 대한 국가 추정치를 이용했으며, 이는 전 세계 인구의 81%를 차지한다. 서비스 이용률은 농촌지역(44%)보다 도시지역(62%)이 더 높았고, 서비스 이용 인구의 3분의 2가 도시 지역에 거주 중인 것으로 나타났다. 기본적인 서비스조차 받지 못하는 사람들의 3분의 2가 농촌지역에 거주하고 있으며, 이 중 절반은(약 50%는) 사하라 사막 이남 아프리카에서 살고 있다.

 
In 2020, 54 per cent of the global population (4.2 billion people) used safely managed sanitation services. National estimates were available for 120 countries and seven out of eight SDG regions, representing 81 per cent of the global population. Coverage was higher in urban areas (62 per cent) than in rural areas (44 per cent), and two thirds of the population using safely managed services lived in urban areas. Two thirds of those who still lacked even basic services lived in rural areas, nearly half lived in sub-Saharan Africa.

2015〜2020년 전 세계적으로 노상 배변을 하는 인구는 2억4천500만 명 감소했는데 이 중 중앙아시아와 남아시아 국가 인구가 1억9천600만 명으로 약 80%를 차지한다. 동부 및 남동아시아와 라틴아메리카 및 카리브해 지역에서는 각각 2천400만 명과 1천만 명이 감소했다. 전 세계 많은 지역에서 2030년까지 노상 배변 근절을 목표로 여러 가지 노력을 기울이고 있지만, 사하라 사막 이남 아프리카는 유독 이행이 더디며 오세아니아의 노상 배변 비율은 도리어 증가했다.

The number of people practising open defecation globally decreased by 245 million between 2015 and 2020, with countries in Central and South Asia accounting for four out of five (196 million). Eastern and South Eastern Asia and Latin America and the Caribbean recorded reductions of 24 million and 10 million respectively. While most regions are on track to eliminate open defecation by 2030, sub-Saharan Africa is progressing too slowly and open defecation rates in Oceania have increased.

2020년 기준 전 세계 인구의 71%(약 55억 명)가 집에 기본적인 손 씻기 시설(비누와 물)을 갖추고 있는 반면, 21%(16억7천만 명)는 물이나 비누가 없는 손 씻기 시설을 이용하고 있었고, 손 씻을 위생시설이 아예 없는 인구도 9%(6억7천만 명)에 달했다. 손씻기 추정치는 8개 SDG 지역 중 4개 지역과 79개 국가의 인구 데이터를 사용했으며, 이들 국가 및 지역 인구는 세계 인구의 50%를 차지한다. 고소득 국가에 대한 데이터는 거의 없었다.

테드로스 아드하놈 게브레예수스(Tedros Adhanom Ghebreyesus) 세계보건기구(WHO) 사무총장은 “손 씻기는 ‘코로나19(COVID-19)’와 같은 전염병 확산을 막는 가장 효과적인 방법 중 하나이지만 전 세계 수백만 명의 사람들이 안정적이고 안전한 물을 공급 받지 못하고 있다”라면서 “우리가 이 대유행을 끝내고 보다 탄력적인 의료 시스템을 구축하기 위해서는 안전한 물, 위생시설, 위생 서비스에 대한 투자가 세계적인 우선순위가 되어야 한다”고 강조했다.

 
In 2020, 71 per cent of the global population (5.5 billion) had a basic handwashing facility with soap and water available at home. Another 21 per cent (1.6 billion) had handwashing facilities which lacked water or soap at the time of the survey, and 9 per cent (670 million) had no handwashing facility at all. Handwashing estimates were available for only four out of eight SDG regions and for 79 countries, representing 50 per cent of the global population, but few data were available for high-income countries.

“Handwashing is one of the most effective ways to prevent the spread of COVID-19 and other infectious diseases, yet millions of people across the world lack access to a reliable, safe supply of water,” said Dr Tedros Adhanom Ghebreyesus, WHO Director-General. “Investment in water, sanitation and hygiene must be a global priority if we are to end this pandemic and build more resilient health systems.”

SDG가 불평등을 줄이는 데 초점을 두고 있는 건 분명하지만, 국가, 지역 및 세계 평균은 종종 국가 간 및 국가 내 서비스 수준의 심각한 불평등을 가리기도 한다. 예를 들어 2020년에는 전 세계 인구 4명 중 3명이 안전하게 관리되는 식수 서비스를 이용했지만, 대륙별 보급률은 96%(유럽과 북아메리카)부터 30%(사하라 사막 이남 아프리카)까지 다양했다.

이러한 격차는 사하라 사막 이남 아프리카의 21개 국가들 사이에서 더욱 두드러졌다. 국가 추정치가 가장 높은 곳은 레위니옹(Reunion)으로 94%였고 가장 낮은 곳은 차드(Chad)로 6%였다. 또한 차드의 국가 평균은 도시와 농촌 간 격차를 가렸다. 실제 2020년을 기준으로 차드의 식수 보급률은 농촌보다 도시가 9배 더 높았다.

차드 정부가 지난 2019년 다중지표클러스터조사(MICS)를 통해 지역 내 접근성, 필요할 때의 이용가능성 및 식수 수질(오염되지 않음)에 대한 지역(sub-region) 간 불평등을 추가로 조사한 결과, 이용가능성과 수질 면에서의 격차는 작았지만 접근성 면에서는 격차가 큰 것으로 나타났다. 접근성이 가장 높은 곳은 42%, 가장 취약한 곳은 4%로 나타났다. 또한 주별(sub-national regions) 접근성에도 큰 격차가 있었는데, 보르쿠(Borkou)주 69%부터 라크(Lac)주 2%에 이르기까지 다양한 분포를 보였다.

 
The SDG targets have an explicit focus on reducing inequalities but national, regional and global averages often mask significant inequalities in service levels between and within countries. For example, while three out of four people worldwide used safely managed drinking water services in 2020, regional coverage ranged from 96 per cent in Europe and Northern America to just 30 per cent in sub-Saharan Africa.

Inequalities were even more pronounced among the 21 countries in sub-Saharan Africa, with national estimates ranging from 94 per cent in Reunion to just 6 per cent in Chad. The national average for Chad also masks a big gap in coverage between rural areas and urban areas, which were nine times more likely to have safely managed drinking water in 2020.

The Chad 2019 Multiple Indicator Cluster Survey (MICS) enables further analysis of sub-national inequalities in accessibility on premises, availability when needed, and drinking water quality (free from contamination). Disparities between the richest and poorest were relatively small for availability and quality but much larger for accessibility, with 42 per cent of the richest and just 4 per cent of the poorest having improved sources accessible on premises. There were also large disparities in accessibility between sub-national regions, ranging from 69 per cent on premises in Borkou to just 2 per cent in Lac.

Achieving SDG targets in fragile contexts will require a dramatic acceleration in current rates of progress

취약성은 SDG를 달성하는 데 커다란 위협으로 작용한다. 2020년 세계 인구의 23%(약 18억 명)와 극빈층의 4분의 3 이상이 취약한 환경에서 살고 있다. 이러한 곳에서 살아가는 사람들의 물 및 위생(Water, Sanitation and Hygiene, WASH) 서비스 수준은 그렇지 않은 환경에서 살아가는 사람들보다 훨씬 낮다. 2030년 SDG 달성을 가속화하기 위한 노력의 주요 초점은 이들이 되어야 한다.

SDG 이행을 시작하고 첫 5년 동안, 취약 지역의 기본적인 식수 보급률은 2015년 71%에서 2020년 74%로 증가했다. 그러나 이들 지역이 기본적인 식수에 대한 보편적인 접근을 달성하기 위해서는 현재의 목표 이행 속도를 4배 높여야 하고, 안전하게 관리되는 식수에 대한 보편적인 접근을 달성하기 위해서는 23배 높여야 한다.

같은 기간 노상 배변 비율은 2015년 16%에서 2020년 13%로 감소했다. 하지만 2030년까지 노상 배변을 근절하기 위해서는 현재의 이행 속도가 두 배로 빨라져야 한다. 또한 기본 위생시설에 대한 보편적인 접근을 달성하려면 7배, 안전하게 관리되는 서비스에 대한 보편적인 접근을 달성하기 위해서는 9배 이상 빠른 목표 이행이 요구된다.

기초 위생 서비스의 보급률은 2015년 43%에서 2020년 48%로 증가했지만, 현재 진행 속도로는 2030년까지 58% 수준밖에 달성할 수 없다. 취약계층의 위생에 대한 SDG 목표를 달성하려면 현재 목표 이행 속도를 5배 더 높여야 한다.

헨리에타 포레(Henrietta Fore) 유니세프(UNICEF) 총재는 “전염병 이전에도 수백만 명의 어린이와 가족들이 깨끗한 물, 안전한 위생시설, 손을 씻을 장소가 없이 고통을 받아왔다”라면서 “지금까지의 놀라운 진전에도 불구하고, 우리의 대응 능력을 능가하는 요구가 계속 이어지고 있다. 이제 모든 아동과 가정에 ‘코로나19’와 같은 전염병 퇴치를 비롯해 건강과 웰빙에 대한 가장 기본적인 요구를 제공하기 위한 노력을 획기적으로 가속화할 때가 왔다”고 역설했다.

Fragility poses a major threat to the achievement of the SDGs. Fragile contexts were home to 23 per cent of the world’s population (1.8 billion people) and more than three quarters of those living in extreme poverty in 2020. People living in fragile contexts often have much lower WASH service levels and will need to be a major focus of efforts to accelerate progress towards the 2030 SDG targets.

Over the first five years of the SDG period, basic drinking water coverage in fragile contexts has increased from 71 per cent in 2015 to 74 per cent in 2020. However, achieving universal access to basic drinking water will require a 4x increase in current rates of progress, while achieving universal access to safely managed drinking water will require a 23x increase.

Fragile contexts have reduced open defecation from 16 per cent in 2015 to 13 per cent in 2020, but rates of progress will need to double to achieve elimination by 2030. Achieving universal access to basic sanitation will require a 7x increase, and achieving universal access to safely managed services will require a 9x increase.

Coverage of basic hygiene services has increased from 43 per cent in 2015 to 48 per cent in 2020, but at current rates of progress only 58 per cent of the population living in fragile contexts will have access by 2030. Achieving the SDG target for hygiene in fragile contexts will require a 5x increase in current rates of progress.

New data show that many women and girls do not have the services they need for menstrual health

SDG 6.2는 여성과 소녀 및 취약계층의 요구에 특별한 주의를 기울일 것을 명시하고 있으며, WASH(Water, Sanitation and Hygiene) 프로그램은 생리 건강과 관련한 요구를 점점 더 모니터링하고 있다. WHO와 유니세프는 JMP(Joint Monitoring Programme, 공동 모니터링 프로그램)를 통해 생리 건강에 대한 새로운 국가 데이터를 포함하도록 글로벌 데이터베이스를 확장했다. 15〜49세 여성을 대상으로 한 가구 설문조사 문항에 생리 건강 및 관련 WASH 서비스 필요성과 관련한 새로운 지표를 포함시켰으며, 이들을 분류하면 다음과 같다.

초경(첫 생리) 전 생리에 대한 인식.
패드, 면, 탐폰 또는 컵과 같이 생리혈을 모으고 흡수하는 생리대 사용. 이들은 또한 일회용 재료와 재사용 가능한 재료로 분류.
집에서 씻고 생리대를 교체할 수 있는 개인 공간 이용.
생리 중 학교, 직장, 사회 활동 등 참여.

이러한 생리 건강 지표에 대한 국가 데이터가 42개국에서 수집됐다. 많은 국가에서 여성과 소녀 상당수가 생리 건강에 필요한 서비스를 받지 못하고 있으며, 취약계층, 특히 하위 국가 지역(sub-national region)에 거주하거나, 장애가 있는 여성과 소녀에게서 그 정도가 심한 것으로 나타났다.

SDG target 6.2 calls for ‘special attention to the needs of women and girls’ and WASH programmes are increasingly monitoring menstrual health related needs. The JMP has expanded its global database to include emerging national data on menstrual health.  New indicators related to menstrual health and associated WASH service needs have been progressively included in household survey questionnaires for women and girls age 15 to 49, and can be grouped as follows:

Awareness of menstruation before menarche (first menstruation).
Use of menstrual materials to capture and contain menstrual blood, such as pads, cloths, tampons or cups. These can also be grouped into single-use and reusable materials.
Access to a private place to wash and change while at home.
Participation in activities during menstruation, such as school, work and social activities.

National data on the above menstrual health indicators have been collected from 42 countries. In many countries a significant proportion of women and girls do not have the services they need for menstrual health and there are often substantial disparities between population sub-groups, particularly between sub-national regions and for women and girls with and without disabilities.
